140 - Dr. Eric Thuler - Tongue Reduction Surgery for OSA

140 - Dr. Eric Thuler - Tongue Reduction Surgery for OSA

Emma is joined by Dr. Eric Thuler, an adjunct Professor in sleep surgery at the University of Pennsylvania.

    In this episode:

    * Dr. Thuler explains how he trained in medicine and got into otolaryngology for his residency.

    * Eric explains research into DISE and CT scans of OSA patients.

    * Research with myofunctional therapists and how tongue positioning could change the airway.

    * Joining the group at University of Pennsylvania to develop a way to measure upper airway collapsibility.

    * Questioning whether AHI is the best measure for OSA when it is really the effort to breathe which depends upon collapsibility.

    * Research into soft tissue volume and how that didn't show a relationship to increased collapsibility. However, they found that increased tongue volume did indicate a higher collapsibility of the airway.

    * Using the Da Vinci system to do less invasive surgery.

    * Looking at narrow and underdeveloped jaws and how that affects whether there is enough room for the tongue.

    * Dr. Thuler explains that obstructive sleep apnea requires choosing the right treatment option for the right patient.

A treasure trove

I was diagnosed with sleep apnea 15 years ago and I thought I knew what I needed to know about the subject. In the last couple of years I have been struggling more with the condition and have made a real effort to dive more deeply into the field. Emma’s podcast is really a great resource for the people to learn more and consider a broader approach to their own treatment.

Binge-worthy!

Just got diagnosed. Thank you for this incredible resource.

A Voice for Patients…

The landscape of Sleep Apnea is fragmented, complex, and intimidating, with many different silos of thought. These days, patients must navigate an increasing array of options for diagnosis and treatment, all with little access to individualized guidance, creating unique existential crises for those “left behind”—both providers and patients alike!

The truth is: there is no “one size fits all” solution for Sleep Apnea. For anyone who has felt “left behind” by the medical or dental establishment, this podcast is for you. Emma is funny, insightful, and humane as she consistently brings the conversation around to the most important player in the Sleep Apnea circus: the patient.

For providers who practice patient-centered medicine: here’s a treasure trove of feedback for how patients suffer from lack of empowerment in our system. Hearing these stories will help you help your patients.
