“移路同行” 专门为方便广大器官移植受者健康管理、方便专家对移植受者健康指导而设计，是中国器官移植发展基金会设计、开发和运维的一个公益项目。
“移路同行”极大地方便了移植受者对日常指标进行记录管理，不仅可以记录、保存并随时查看历次随访检查结果，还可以交流相关知识、分享诊疗经验、咨询相关问题。
中国器官移植发展基金会希望通过“移路同行”的应用，进一步提高我国移植受者健康自我管理意识，帮助移植受者保存并利用好移植随访数据，进而达到促进移植受者康复和中长期健康生活的目的。
